Donepezil (Aricept®)
 
Donepezil is a medication that inhibits acetylcholinesterase, a key enzyme responsible for breaking down acetylcholine, a neurotransmitter critical for memory, learning, and cognitive function. By preserving acetylcholine levels in the brain, Donepezil enhances communication between nerve cells, helping to support mental performance and daily activities in patients with mild to severe Alzheimer’s disease.
 
Rivastigmine (Exelon®)
 
Rivastigmine acts on two cholinesterase enzymes, acetylcholinesterase and butyrylcholinesterase, boosting acetylcholine levels in the brain. This dual action improves cognition, attention, and memory, and is used to manage symptoms of Alzheimer’s disease as well as Parkinson’s disease dementia.
 
Galantamine (Reminyl®)
 
Galantamine is indicated for mild to moderate Alzheimer’s disease and works by preventing acetylcholine breakdown while also enhancing nicotinic receptor activity. This dual mechanism strengthens neural signaling, supporting memory, learning, and overall cognitive function.
 
Additional Benefits:
Clinical trials show that Donepezil, Rivastigmine, and Galantamine may also benefit individuals with dementia with Lewy bodies, Parkinson’s disease dementia, and mixed Alzheimer’s and vascular dementia. While all three drugs are similarly effective, Rivastigmine may be preferred if hallucinations are a prominent symptom.
 
Common Side Effects:
Patients may experience mild nausea or decreased appetite, usually resolving within two weeks of treatment.
 
Memantine (Namenda®)
 
Memantine is recommended for moderate to severe Alzheimer’s disease, dementia with Lewy bodies, and mixed Alzheimer’s and vascular dementia. It is particularly suited for patients who cannot tolerate acetylcholinesterase inhibitors. Memantine works by regulating excessive glutamate in the brain, preventing neuronal damage and supporting cognitive function.
 
Common Side Effects:
Temporary side effects may include headaches, dizziness, or constipation, which generally subside with continued use.
